I found the tutorial for adding the boot restore floppy for PQimage to UBCD, but it is not written for UBCD 4.1 and no longer seems accurate.

OK, I've had a quick look at the tuto you gave a link to, and the only thing that seemed not applicable was the method to create the ISO.
Now you should use ubcd2iso instead.

If you need a batch file that will do it for you..let me know and Ill walk you through the steps of making one. The point of making one is you just double click it and it does all the work for you so you dont have to go cmd prompt to created the iso.
