Just a small problem - I tried customizing UBCD v4.0 earlier by following the instructions here, however the location stated for custom.cfg ("c:\ubcd\menus\custom.cfg") appears to be wrong. The CD actually looks for this in custom\custom.cfg (therefore should be - "c:\ubcd\custom\custom.cfg").
Might be worth updating the instructions to reflect this (unless of course the intention is to keep it in the 'menus' directory as it used to be)..

Nope, as far as I know, you can't boot an ISO image directly.
For certain ISO images (eg. isolinux-based ones), you can get around by extracting its content and using the "kernel" command to load it. A few apps that previously existed as ISO images in UBCD V3.4 are now launched this way.
If you have difficulties with any ISO image, post its name here and we can discuss.
