Adding 'raw' as parameter to MEMDISK, solved the "Invalid Opcode" message:

UBCD v4.10 is very old, try a newer version.

You can try TestDisk to search for lost partitions.

Did you try testdisk? It is able to search for deleted partitions and fix the partition table.

You should install Syslinux 4.02 on your USB stick.

Did you run ubcd2usb with admin rights?

Try testdisk (Parted Magic) to recover lost partitions.

You can also try to boot to Parted Magic first. When Parted Magic is fully booted, insert your USB stick.
Then run the following command: It should show some USB related messages, if your USB stick is recognized.

Most of those tools that can read and/or write from/to NTFS are quite buggy.
Using a linux distro with ntfs-3g support (almost every distro has it), is the best way to access (read/write) NTFS filesystems.
You can use Parted Magic (included in UBCD) to do that.

It can also mean that your download is corrupted. You can try to redownload the file from the same mirror or a different one. If you need Parted Magic, it might be a good idea to just download the last version.
You can install it to an UFD with unetbootin too, if you don't want to burn a CD.

Try the vesa menu option in Parted Magic.

- Original ISO, customised ISO, installed on USB
